  正文: 生理学模型先假设生理性觉醒和睡眠是相对的,理论是慢性失眠可能是一种状态,在这种状态中,患者处于某种特殊水平的觉醒,或者某个特定的睡眠期之中或之前的觉醒状态。 The physiological model assumes that physiological wakefulness and sleep are relative. So in the theory, chronical insomnia could be a state, in which the patient is in a relatively wakefulness, or a state of wakefulness during or before a specific sleeping stage.

  正文: 和失眠的生理模型一样,失眠的认知模型同样认为失眠的发生同觉醒有关,觉醒和睡眠相互对立。不同是认知模型的核心理念是,认知觉醒以思虑和担忧的形式,导致患者容易失眠,诱发急性发作,失眠状态持续下去就导致慢性失眠。认知模型的三个主要部分组成易感个体、诱发因素、持续因素。 Just as the physiological model, cognitive model of insomnia similarily thinks it relevant to wakefulness, and wakefulness is opposite to sleep. Differently, the core idea of cognitive model is that cognitive wakefulness leads to and induce accute insomnia in the form of anxiety, losing sleep lasts and finally become chronical insomnia. Susceptible patient, inducing factors and continual factors comprise the main parts of cognitive model.

  药物:病人会首选的治疗方法,省事,简单,不用谈及心理隐私。急性期使用较好,是一种对症治疗方法。Drugs: first choice to the patients for it's convenient, simple, and no privacy involved. Good for acute stage. Belongs to a symptomatic treatment.

  补充替代疗法

  CBT:无论是长期、短期、单独使用还是联合使用,都非常有效。被推荐为首选的治疗方法。一般治疗比较彻底,无副作用。Very effective for chronial, shortterm, single and conjunctive use. Reccomended first choice for it's thorough and without side effect.

  针灸、按摩 acupuncture, massage 气功、太极 qigong, taiqi 中药、植物药 Chinese medicine, herb medicine

  正文: 1、轻中度失眠首选心理疗法—CBT the first choice for light and mild insomnia--CBT (1)觉醒时间减少 Reduce the wakefulness. (2)睡眠效率提高 Sleep effectivity increases. (3)总睡眠时间和主观睡眠质量均有提高。Total sleep time and subjective sleeping quality increase. (4)慢波睡眠(SWS)和快速眼动(REM)睡眠的持续时问均有明显增加。SWS and REM increase. (5)维持时间较长,至少可维持8个月。Maintain for more than 8m. (6)患者的主、客观感觉之间的差异缩小,不良认知得到了纠正。The disparity of feeling and real-condition narrows. Wrong ideas get modified. 存在问题problems:显效时间比较长,很多患者在治疗初期焦虑情绪严重,若病情未得到改善,会加重心理负担,对治疗失去信心,这对CBT治疗的进程提出了考验。It takes a relatively long time to show efficacy, many would lose confidence for their anxiety of no improvement and then drop out.

  正文: 2、重度失眠主张综合疗法:Integreted method is reccomanded for severe insomnia CBT联合药物会增加治疗效果,尽管比较小,主要表现为增加睡眠时间。在6周的治疗后,60%的病人有效,42%的病人缓解;在6个月的延续治疗后增加到 65%和51%。在唑吡坦停止使用后CBT联合唑吡坦组,在初始治疗6周阶段取得了更好的长期结果。在疗效的维持上,结合CBT明显优于单用药物治疗,且利于帮助患者撤掉安眠药物。CBT combined with drug will enhance the efficacy, though slight, mainly in sleep extension.
